DO I NEED A DOCTOR’S REFERRAL?

Anyone is able to initially start services at AWAKEN Performance Rehab WITHOUT a doctor's referral. However, if you are coming for PHYSICAL THERAPY services following pain or an injury, then you will need a referral for continued treatments after 10 visits or 30 days from the initial session. Clients receiving wellness, prevention, or performance services do NOT need a doctor's referral.

WHAT SHOULD I EXPECT AT AWAKEN?

All patients and clients should expect a welcoming, fun, and professional atmosphere where they feel cared for, supported, and listened to. AWAKEN provides only the highest level of customer service not often seen in the current medical field and combines high-tech, advanced equipment with cutting edge treatment options. AWAKEN also provides complimentary coffee, tea, hot chocolate, and water bottles in order to improve the patient/client experience.

WILL I BE SCHEDULED AT THE SAME TIME AS OTHER PATIENTS?

ALL physical therapy treatment sessions at AWAKEN Performance Rehab are scheduled so that there is only ONE patient with ONE Doctor of Physical Therapy. Most in-network, commercialized PT clinics will schedule as many as 4-5 patients per therapist per hour; however at AWAKEN, we know the quality of service that we could provide with our brain split 4-5 ways versus when our attention is focused on one individual at a time!

DO YOU ONLY WORK WITH ATHLETES?

AWAKEN Performance Rehab is passionate about helping any individual who is suffering from an orthopedic-based pain or injury. We specialize in treating a variety of different injuries and conditions, and we strive to bring the same elite-level care to each and every patient.

DO YOU TAKE MY HEALTH INSURANCE?

AWAKEN Performance Rehab is considered an out-of-network provider and payment is required by each patient via cash, credit card, or HSA (Health Savings Accounts)/FSA (Flexible Spending Accounts) prior to sessions. AWAKEN Performance Rehab is able to produce a "super bill" for patients who wish to submit the payments towards an out-of-network deductible. MOST patient's health insurance plans do have out-of-network benefits that they aren't even aware of!

DO YOU TAKE MEDICARE?

Unfortunately if you have Medicare and are seeking true PHYSICAL THERAPY services, then Medicare will only let you go to a clinic that is in-network with Medicare (we would be more than willing to assist you with finding the best providers). HOWEVER, if you are seeking WELLNESS/PREVENTATIVE services that your insurance would not cover, then we would likely be able to work together.

WHEN SHOULD I ARRIVE FOR MY SESSION?

Please arrive at least 5-10 minutes prior to your scheduled time each appointment in order to maximize your whole session. Due to the fact that we only work with our patients one-on-one during Physical Therapy sessions, we have to stick to a tight schedule throughout the day and we do everything in our power to get patients/clients started on time.

WHAT SHOULD I WEAR TO AWAKEN?

Please feel free to wear clothing that you would feel comfortable moving around and exercising in. In order to perform the most accurate assessments and targeted treatments, the patient's clothing should ideally be loose enough to provide access to the skin in region of discomfort. AWAKEN Performance Rehab does have a private, single bathroom available for changing upon arrival for your session. During the winter season, please bring a change of footwear in order to maintain cleanliness of the clinic floor.
